A little after 3:30 on Sunday afternoon, Yu Zhiming, his eldest sister Yu Zhaoxia, and his niece Fu Xiaoxue arrived smoothly at Binhai South Station aboard the G211 high-speed train.
They were hauling two large suitcases and wearing two backpacks, and had only taken a few steps from the train when someone called out to them.
The person who stopped them was a stout young woman, her figure nearly the same width from top to bottom.
With her was a middle-aged woman in her forties or fifties.
"Doctor, I’m so sorry to bother you."
The young woman hurried over, dragging a small suitcase with one hand and clutching a large manila envelope in the other, an apologetic look on her face.
"Doctor, when you stepped up to help on the train, I just knew you were a kindhearted doctor, a good doctor..."
The help she was referring to had occurred just before the train reached Binhai, when an announcement suddenly came over the PA system.
It said a six-year-old child was coughing up blood, a frightening sight, and requested medical assistance.
As a passenger on the train, Yu Zhiming had naturally stepped forward.
Upon examination, he discovered that a foreign object had pierced the child’s throat but hadn’t damaged any blood vessels.
While it looked like the child was coughing up mouthfuls of blood, it was merely some blood mixed with saliva. The parents had just been overly worried.
Using a pair of eyebrow tweezers provided by the child’s mother, Yu Zhiming easily removed the foreign object.
A small, sharp piece of a sunflower seed shell...
Yu Zhiming cut off her flattery and asked directly, "How can I help you?"
The young woman gave a sheepish smile. "Doctor, I’ve come to Binhai to see a doctor."
"I’ve been married for four years and haven’t been able to get pregnant."
"Over the last year or two, I’ve been to several hospitals and spent a lot of money, but nothing has worked."
The young woman held out the thick manila envelope, her face full of expectation. "Doctor, these are my medical records. Could you take a look for me?"
To be honest, Yu Zhiming didn’t want to get involved in this kind of non-emergency medical consultation.
’People are so unpredictable.’
The cautionary tale of being beaten to a pulp was still fresh in his mind.
So, facing the woman’s request, Yu Zhiming politely declined. "I’m sorry, but I’m not a gynecologist. I can’t give you a professional diagnosis."
"Since you’ve come to Binhai, I suggest you go to one of the major hospitals here and consult a gynecology specialist."
Fu Xiaoxue, who was standing to the side, chimed in, "Huashan Hospital is one of the four famous hospitals in Binhai. They have excellent medical resources. I suggest you go there."
Yu Zhiming shot a look at his talkative niece as the young woman asked, "Is Huashan Hospital better for gynecology than East Sea Infertility Hospital?"
Her question left Yu Zhiming and the others speechless.
Fu Xiaoxue couldn’t help but reply, "Ma’am, East Sea Infertility Hospital... just from the name, you can tell it’s a private quack hospital."
"Hospitals like that are only out to make money. How could their medical standards possibly be any good?"
"You didn’t actually fall for their advertisements, did you?"
The young woman said awkwardly in a low voice, "I found out about one of their medical experts on a livestream."
"That person seemed really impressive."
She looked at Yu Zhiming and asked, "Doctor, is East Sea Infertility Hospital really that bad?"
"Are their ads just a scam to trick people?"
Yu Zhiming gave a faint smile and replied vaguely, "I’m not familiar with it, so I can’t say that East Sea hospital has poor standards."
"I can only say that Huashan Hospital, which my niece recommended, and the other three major hospitals of its caliber, will very likely be able to solve your problem."
The young woman nodded. "Doctor, you’re a good doctor. I trust you!"
"I’ll go to Huashan Hospital."
Meeting the young woman’s trusting gaze, Yu Zhiming noticed that although she was plump, her complexion was somewhat dull, and her hair was a bit dry.
His heart softened a little, and he couldn’t help but ask, "Have you always been this heavy?"
The young woman smiled shyly. "When I got married, I was still quite slim, less than 125 pounds."

As soon as she finished speaking, the middle-aged woman beside her muttered under her breath, "Lazy and sleeps all the time. It’d be a miracle if she lost any weight."
The middle-aged woman’s voice was very soft, almost a whisper, but Yu Zhiming heard it clearly.
This sparked a few suspicions in Yu Zhiming’s mind.
His curiosity piqued, he reached out and took the manila envelope from the woman’s hands to verify his theory.
The envelope contained her medical history, various physical examination and lab reports, and imaging scans of her pelvic and abdominal area.
Yu Zhiming glanced at the medical facility listed on the reports—Angel Infertility Hospital—and shook his head to himself.
’This poor woman,’ he thought. ’A classic case of getting fleeced over and over without ever learning her lesson.’
Yu Zhiming quickly flipped through the medical history, lab reports, and scans, finding no organic lesions in the woman’s uterus, ovaries, or fallopian tubes.
Her various hormone levels, though fluctuating, were mostly within a reasonable range.
The only exception was her PRL (prolactin) level, which was high, with a serum concentration of 95.
This elevated level could be caused by certain medications or her physiological cycle, but in a way, it also seemed to confirm Yu Zhiming’s suspicion.
The diagnosis in her medical history was irregular menstruation.
Yu Zhiming looked through the documents one more time but couldn’t find the specific test result he was looking for. He put everything back into the manila envelope and returned it to the woman.
He chose his words carefully. "When you go to the hospital for a check-up this time, ask them to screen you for a pituitary adenoma."
"Pituitary? Adenoma? A tumor?" The young woman’s eyes widened, and her breathing grew ragged.
"My infertility is because of a tumor in my brain?"
"That sounds a little far-fetched, doesn’t it?"
Yu Zhiming quickly clarified, "Don’t worry. It’s just a screening to rule it out. I’m not saying you have a brain tumor."
He explained further, "Based on all your test results, your reproductive organs are fine."
"Your infertility is likely due to hormonal reasons."
Yu Zhiming pointed to his own head and continued, "There’s a small organ in our brain called the pituitary gland. It’s responsible for regulating the secretion of most of the hormones in our bodies."
"I noticed that among all the tests you’ve had done, a check of your pituitary gland is missing."
"I suggest you get that one done."
The young woman, only half-understanding, let out a soft "Oh." She forced a smile and said, "Okay, Doctor, I’ll listen to you. I’ll ask the doctor to do that... pituitary check."
"Doctor, sorry to have bothered you. Goodbye!"
With that, the young woman and her middle-aged companion walked away briskly.
Fu Xiaoxue watched their retreating figures and snorted. "Uncle, a brain problem causing infertility? They probably think you were being an alarmist."
Yu Zhiming just chuckled softly.
Given the woman’s situation—years of infertility—the psychological pressure must have been immense.
That much was clear from her dull complexion and dry hair.
But for her to remain so heavy, with her weight staying stable or even increasing under such long-term stress, was somewhat contrary to common sense.
Combining this with her various test results, Yu Zhiming inferred that there was a high probability the woman had a pituitary problem.
For example, a pituitary PRL-secreting adenoma.
It is a type of functional pituitary adenoma, most common in women of marriageable and childbearing age, from twenty to forty years old.
Female patients primarily present with irregular menstruation, infertility, and an increase in subcutaneous fat, often accompanied by symptoms like fatigue, lethargy, drowsiness, and headaches.
This matched some of the woman’s physical signs...
Dragging his luggage, Yu Zhiming chatted idly with Fu Xiaoxue as they leisurely made their way to the station exit.
The first thing that caught their eye was a large sign: "East Sea Infertility Hospital Pick-up Point."
Fu Xiaoxue said in surprise, "Are hospitals this competitive now? They’ve started coming to the station to pick up patients?"
She peered at the area for a moment. "I don’t see those two. I wonder if they took your advice, Uncle?"
"It’s up to them."
Yu Zhiming replied casually, his eyes having spotted the smiling face of his Fourth Sister in the welcoming crowd...
