The Enhanced Doctor
Chapter 1377: Raising the Difficulty for Myself
Chapter 1377: Chapter 1377: Raising the Difficulty for Myself
"You really have some guts to take on this job."
Chen Xuehai said.
"I can’t help it; what the patient said really moved me. Isn’t that right, Liu Yiqing?" Liu Banxia asked.
"Teacher Liu, you’re teasing me again. What’s the success rate of the surgery?" Liu Yiqing asked.
"Well, as long as Old Wang next door puts in some effort, I think it shouldn’t be a problem." Liu Banxia said.
"It’s just that the process will be quite troublesome; the patient has a lot of feces in the intestines, so we’ll need to be cautious about it."
"Normally, even if his tumor is a bit large, about two hours should be enough. But now, because of the fecal content, it’s hard to control the time."
"We’ll see the effect of the nurses’ prep once the tests are done. But it’s likely there will be a lot of residue, so let’s estimate the surgery time to be around three hours."
"Are you so sure that his children will dare to approve of the surgery?" Wang Lei asked.
"I think it’s quite likely. In a serious sense, this might be the patient’s last wish. The risk of a TaTME procedure is mainly just the length of surgery." Liu Banxia smiled and said.
"And, by taking this risk, you’ll fulfill many of the patient’s wishes. Actually, if you think about it, will they agree to it? Will they take that risk?"
Listening to him, those around couldn’t help but nod.
That’s also true; there’s always risk in surgery. The patient’s congestive heart failure won’t improve with this kind of operation either.
The primary risk of this surgery is similar, so when the patient’s family asked about the success rate, Liu Banxia didn’t give a definitive answer.
It’s really hard to say, unlike routine surgeries. Even routine procedures come with certain risks.
"Doctor, we’ve decided, we’ll go for that non-invasive surgery." At this point, the patient’s son walked over.
Liu Banxia nodded, "Li Hao, explain the surgery process to them, then arrange for an enema after the tests are complete. If everything flows smoothly, it would be ideal, making surgery time-saving."
Li Hao quickly led the patient’s family away to not miss even a single step of the procedure.
"Sigh... going to be late finishing work today again. Old Wang, your hard work is appreciated." Liu Banxia said.
"In this line of work, the times I’ve gotten off work on time have been limited. Thought I could be off on time today, you’d better be swift in your work." Wang Lei prompted.
If it were anyone else, he wouldn’t have said this, knowing Liu Banxia’s skills. If Liu Banxia works skillfully, indeed, it can save some time.
Liu Banxia also picked up his phone, calling Qiaoqiao, to inform her that his return home would be uncertain tonight.
Everyone hopes the surgery will proceed smoothly, but when it’s underway, all sorts of situations can arise.
Qiaoqiao has gotten used to it; to be precise, even if she’s not used to it, there’s nothing she can do. It’s not subject to her will; if there’s a late shift, there must be a late shift.
"This surgery might be somewhat complex, so you’ll all have to put in extra effort. The most crucial part is ensuring sterility and lack of tumors, especially since this patient’s intestines can’t be prepped in advance." Liu Banxia said.
"Teacher Liu, don’t worry, we can handle these tasks." Miao Rui said.
"That’s better, showcase your charm and get the blood test results faster. Get the other imaging results out ahead of time, and also find out who’s on duty tonight in cardiology." Liu Banxia said.
"Teacher Liu, are you worried about the patient’s heart issues?" Liu Yiqing asked.
Liu Banxia nodded, "It’s sort of a backup plan; if at any moment we can’t manage it, we’ll need to quickly call the technical advisor."
"Although I estimate it’ll just be the emergency measures that might come into play, nothing else. You should still keep asking around, and if anything happens, you’ll handle it."
"Okay."
Liu Yiqing responded earnestly, then ran over to cardiology.
It seemed that potential incidents were prepared for, Liu Banxia then sat aside, started contemplating the patient’s surgical plan.
Theoretically, it is considered routine surgery, albeit slightly altered. But it doesn’t substantially increase the difficulty of surgery, only makes it a bit longer.
The most critical factor is the patient’s heart condition, the rest is luck. See the patient’s luck, as well as the leading surgeon’s luck.
This idea seems elusive, yet many times the complications in surgery are down to luck, aren’t they?
At 3:30 p.m., Li Hao came over, "Teacher Liu, we’re all set, and the patient has been brought to the operating room."
"OK, let’s go up. Stay focused and aim to finish in under three hours." Liu Banxia said.
Although they often went on the stage with Liu Banxia, even for this patient, there was still some nervousness.
Unlike the usual patients whose surgeries have clear indications, this one just wants to live a little more comfortably in the remaining days.
Arriving at the operating room, Miao Rui had already completed all the preparations.
"Don’t worry that our surgery seems a bit rushed; the quality won’t be poor. Leave the rest to us. You just focus on how to enjoy yourself by then, that’s enough," Liu Banxia said to the patient.
"I’ll take a little nap first, and soon enough, I’ll have endless sleep," the patient said with a smile.
Liu Banxia nodded at him and then gave Wang Lei a nod as well.
Wang Lei wasted no time and proceeded with the anesthesia.
"Old Wang, our goal is to wrap things up in three hours. Are you confident?"
After successful anesthesia, Liu Banxia asked.
"Absolutely, anyway, Sister Li said to time it perfectly for cooking. It’s up to you whether I eat something hot or cold when I get home," Wang Lei replied.
"Good man, putting all the pressure on me, huh? Li Hao, let’s get started," Liu Banxia said.
Li Hao quickly got to work.
Knowing that time is precious for the patient, the longer they lie on the table, the greater the risk.
"Currently, vital signs are very stable," Wang Lei reported.
"OK, Miao Rui, tell us which removal method we’re using for this surgery. Eversion, extraction, or excision and drag? I’ll follow your lead," Liu Banxia asked while operating.
"Teacher Liu, you can’t stump me now. This patient has mid rectal cancer, and typically, an extraction excision is the way to go," Miao Rui answered.
"However, due to the tumor’s large size, there might be some risks with exterior pulling, so excision and drag is more suitable. It’s a common approach for large tumors, although a bit more challenging to perform, it offers better prognosis for the patient."
"Impressive, knowledge is so extensive now? Looks like I need to put in some effort. Take it slowly, no rush," Liu Banxia quipped.
"Liu Yiqing, what are you doing?"
After finishing, he suddenly shouted.
"Huh? I’m not doing anything?"
Liu Yiqing responded quickly.
"Then why don’t you explain to the team the differences between these three excision methods? I’m worried you might get sleepy," Liu Banxia said.
"Actually, there are many ways to remove a specimen, but essentially, these three are the most practical, and can handle many tumor scenarios," Liu Yiqing explained.
"To summarize, it’s internal versus external excision. The excision and drag method we chose this time is an internal excision, usually used for some high tumors and colon tumors."
"If it involves eversion or pulling out, then it’s quite troublesome with such a long intestinal tract, even tricky for Teacher Liu to handle."
Hearing Liu Yiqing, the people in the observation room chuckled. They knew this was Liu Yiqing’s weak counter, even if it seemed feeble, she had to fight back a little.
"The external excision methods are eversion and pulling out, really just as the words mean. The former for low, the latter for mid-location tumors," Liu Yiqing continued.
"But for our Teacher Liu, he tends to use internal methods more often. Even if there are some low rectal cancers, he would choose this way."
"What’s the reason? He’s skilled, not afraid of the complexity during operation. We, on the other hand, should stick to the rules."
"This guy, like performing a stand-up with little chats bundled up," Liu Banxia remarked.
"I noticed some new faces today, so I had Liu Yiqing go over it again. For the old spectators, this is already ingrained in them,"
"Moreover, when performing similar surgeries, we must keep to the principle of sterility and tumor-free conditions. This patient’s situation is quite special; even after an enema, there’s still fecal residue in the intestine."
"Thus, we need to pay extra attention when handling the intestines, especially in the washing step, which must be done safely. Otherwise, if the abdominal cavity is filled with water, we might have to puncture the abdomen."
"Normally, this patient would require laparoscopic assistance, but we can try it first, and if it doesn’t work, then use laparoscopy. Everyone, give me your primal strength."
Li Hao and the rest felt a bit resigned, sensing that Liu Banxia’s mindset was quite relaxed now.
Being so at ease with such a challenging surgery indicates his mindset is rock solid. He’s evidently very confident in the related procedures for this surgery.
Otherwise, he wouldn’t be raising the difficulty himself, and others wouldn’t approach it this way.
You can’t help but be impressed; even working alongside Liu Banxia in surgery, you never know when he’ll surprise you.
Of course, how well this surgery will be performed depends on the subsequent operations.
Nothing is certain just because you’re confident; if it were that easy, confidence would be invincible.
After finishing, Liu Banxia said nothing more; today, time is of the essence. Even if those outside observe what seems a bit hurried, that time still has to be seized.
